Welcome to Innate Health. I am honored to accompany you on your journey towards better health.

To begin, I would like you to be fully informed about the nature of my practice and welcome any questions you may have after reading my practice overview.

WHAT WE OFFER

Dr. Mausumee Hussain, MD, MS (“Dr. Hussain”) is Board certified in Internal Medicine. She practices Integrative Medicine at Innate Health Integrative and Internal Medicine (Innate Health). The National Center for Complementary and Integrative Health (NCCIH) describes Integrative Health care as bringing conventional and complementary medical approaches together in a coordinated way. Dr. Hussain integrates the practice of Internal Medicine with her complementary education and training in functional medicine, nutrition, medical acupuncture, ayur-veda and cupping therapy. Dr. Hussain and Innate Health (we/us/our) aim to deliver a highly personalized and detailed approach to your healthcare, partnering with you to empower you in achieving your health goals.


Our Schedule of Services and Fees:


· Supplements

A core approach of our practice is beginning with lifestyle and dietary change. Supplements are not necessarily required for everyone to complement these changes nor would it be appropriate for certain individuals. In the situations where supplementation is thought to be beneficial, Dr. Hussain will make specific recommendations based on her knowledge, experience and judgement. She recommends therapeutic grade supplements which often differ greatly in their efficacy as compared to over the counter supplements. The reason for this is that the supplement industry is not regulated by the Food and Drug Administration (FDA) and the quality of products can vary significantly based on the individual supplement company’s internal compounding and manufacturing practices.

For your convenience, you may purchase recommended supplements through Innate Health, however, you are in no way obligated to do so and are free to purchase them elsewhere without any change in your rights within our practice. We price the supplements at significantly below MSRP and maintain a low mark-up on our cost in order to cover our expenses in providing this service.


· Communication

We make every effort to respond to your queries as soon as possible during working hours. On occasion, it may take us 48 hours to respond to you. Clinical support or questions are attended through the patient portal only.


· Privacy

While we are not required to follow the privacy requirements under the Health Insurance Portability and Accountability Act (HIPAA), we do respect your privacy and will only release medical information about you as permitted or required by law.

WHAT WE DON’T OFFER


· Primary Care Services

Unfortunately, we do not undertake Primary Care at Innate Health. Although Dr. Hussain may address issues affecting your general health and may prescribe treatments for incidental conditions that arise during your health assessments, the practice is focused on a complementary, holistic or integrative approach to medicine.

You are advised to be under the concurrent care of a conventional Primary Care physician and appropriate subspecialists. This is extremely important because our practice is exclusively office-based and is not affiliated with a hospital. Should you become so ill that you require hospitalization, it is vital that you have a primary care or other physician with hospital admitting privileges familiar with your health problems and history.


· Emergency and Out of Hours Service

We do not have coverage outside of office hours or during office closures. For questions regarding treatment or investigations please call 913-521 3335 during office hours or contact us through your patient portal account. We do not provide emergency or on-call assistance. Please call 911 if you believe you have a medical emergency.


· Guarantees

At Innate Health, we are committed to transparency. It is important that you realize no practice of medicine is an exact science, and we make no guarantees as to the results of any examination, diagnoses, care, or treatment at our practice.



WHAT WE ASK OF YOU

· Patient Responsibilities

Although we will frequently check with you, it is your responsibility to inform us of the following on a regular basis:

1. The name and contact details for your primary care physician and your treating subspecialists;

2. The diagnoses and treatments you have received or are undergoing;

3. Your current medications and supplements; and

4. If you are pregnant.

It is also important that you update your primary care physician regarding the recommendations and treatments received through Innate Health.

We ask that you arrange to visit us at least once a year; If it has been 3 years since your last visit, you will be asked to renew your registration and schedule an initial visit for 90 minutes.


· Authorization and Consents

For healthcare

We will ask you for general consent for medical and healthcare treatment. In addition, we ask for your signed informed consent before specific treatments once we have discussed them with you. This will give you a chance to accurately assess whether you would like to proceed. We do expect you will ask for clarifications should you require them, regarding any aspect of your treatment. Our practice is not required to obtain your prior consent in an emergency, or when there are substantial communication barriers.

For communication

We will also ask for your consent regarding the methods we may use to communicate with you for exchanging clinical information, reminders or logistical communication and for marketing or community announcements.

For medical records

With regards to medical records, due to the potentially sensitive nature of the information that makes this material protected, you will need to provide us written authorization prior to our release of any of your records to your other healthcare providers and prior to our obtaining your medical records from your other providers.


· Financial Responsibility

Innate Health and Dr. Hussain provide direct payment fee-for-service consultations and treatments. Dr. Hussain does not participate in any insurance plans, including Medicare and Medicaid. Consequently, we do not take insurance assignment, which means that payment in full will be required with each visit. Our financial policy is outlined below:

1. Payment is due at the time of service and may be made with cash, credit or debit cards only.

2. A $100 deposit is required at the time you book your first appointment and will be collected through the patient portal via debit or credit card. The $100 booking deposit is applied towards the total fee for the first appointment at the conclusion of the visit. It is non-refundable.

3. Credit and debit card details are maintained in tokenized form and point to point encryption is utilized at all times for your security. This means that we do not store your card details in physical records or on computers in our system.

4. The booking deposit is forfeited in the case of first appointments that are missed without providing 24 hours’ notice.

5. We reserve the right to charge your card should payment not be completed at the time of service or if subsequent appointments are missed without 24 hours’ notice. A $50 fee will be charged for subsequent missed appointments without 24 hours’ notice.

6. Late arrivals beyond 5 minutes for appointments that are scheduled for procedures are automatically cancelled and subject to a $50 cancellation fee.

7. We do not issue refunds for any rendered services or prepaid packages.


· Late Arrivals and Appointment Cancellations

If you are due to arrive late for an appointment, we ask that you contact us as soon as possible to notify us. You can call us at 913-521 3335, or email us at Info@InnateHealthMD.com. Although we will still see you, your appointment will be shortened to end at the originally scheduled time and you will be charged for the length of the originally scheduled visit.

An exception to this is in the case of appointments for procedures which must be rescheduled if you arrive more than 5 minutes late. In this case, the appointment will be canceled and you will be responsible for the $50 missed visit fee according to our financial policy. This is because a treatment, e.g. acupuncture once started, cannot be curtailed early in order to end at the scheduled visit time if you are late to start. It will still require a fixed amount of time to be effective.

With regard to cancellations, we ask for 24 hours’ notice. We respect both your time and our own. In the event that we do not receive at least 24 hours’ notice from you and your appointment is missed, we reserve the right to retain the $100 booking deposit for a first visit, and charge $50 if the appointment was for a subsequent visit.


· Insurance Coverage

We advise all patients to be fully and actively covered by health insurance plans or policies. The services we provide are not intended in any way to replace current or future health insurance coverage.


· Private Contract for Medicare Beneficiaries

Dr. Hussain has opted out of Medicare. This means Medicare beneficiaries must enter into a private contract to participate in care offered by Dr. Hussain at Innate Health. If you are a Medicare beneficiary, please click here to view this private contract. You will be asked to sign it during the registration process.



 

ABOUT INSURANCE CLAIMS

· Superbills

You may request a paper copy of a superbill showing the cost and nature of services provided, however, it will be your responsibility to submit these paper claims to your insurer. Our provision of a superbill is provided as a courtesy only and should not be construed as a representation that any fees paid for services or treatments are covered by health insurance or any other applicable third party payment plans. We reserve the right to decline this provision and to charge a small administrative fee for this service.

· Possible non-coverage of services

Please note that even after you submit the paper copy of the superbill we provide you, insurance reimbursement may not be available. Insurance companies may not pay for office visits where the focus of the consultation is on wellness, herbal medicine, or other complementary and alternative ("CAM") medical services. Some of the lab tests that are ordered, particularly those that are used in support of wellness consultations or are kits sent to labs using innovative approaches to diagnostics, also may not be reimbursed.

· Claim management

You are responsible for knowing your insurance benefits. We may offer some assistance, but given the uncertainty that pervades insurance decisions, we cannot be responsible for any information that turns out to be incorrect. We will endeavor to respond to insurance requests for information within our limitations, but we will not be obligated to take action on your behalf against an insurance carrier for collecting or negotiating your insurance claim. Patients may be charged for responding to requests for information.



Minimizing our Carbon Footprint

Please note that should you choose to join our practice, you will be asked to complete registration forms online. Completed paper copies will not replace this process. If you require help with registration or completing any of our online forms, please call us to make an appointment to come in at least one week before you wish to be seen, so we can assist you with the process.
